Form Required: USPS Form 3813 or 3813-P (for items insured more than $50). Complete and attach the form above the delivery address and to the right of the return address.
Other Requirements: You must present Insured Mail packages at the Post Office™ or to a rural mail carrier. Mail insured for $50 or less must be stamped with the proper insurance endorsement at the Post Office.
